For just $99 per year, your entire household is covered under the Qavvik emergency medevac plan. We believe that everyone deserves access to emergency medical transport without the fear of unexpected bills. This policy provides relief from costs for medically necessary air or ground ambulance services, regardless of pre-existing conditions, insurance status, or age. Coverage starts the moment payment is received and continues for a full year.

Qavvik Air Individual Membership, Summary of Terms and Conditions
100% Native-owned and locally led, Qavvik Air was born out of the desire to provide safe, reliable, and efficient air ambulance transportation to the NANA Region.
- Membership Applicability. Membership is open to any individual and household members listed on their account. Membership applies to myself as the “Primary Member” as well as to members of my “Household” (both defined below). “Household” and “Household Members” include any person or persons who reside under one residential roof at the address listed on the application. In the event a member lives in a duplex or apartment complex, we designate the primary member’s residence/address including the apartment number, not the entire building. Undergraduate college students may be covered under their parent’s membership, as well as anyone residing in the household at the time of application who is later moved to a permanent care facility. Household Members must be listed on the Membership at the time of application or renewal. In order for Membership to apply to anyone who joins my Household after the Membership goes into effect, I must provide Qavvik Air with written notice of the new household member’s name, date of birth, relationship to me, and any primary insurance information. Membership for any household members that are added will not become effective until I notify Member Services in writing of the addition. Newborns are automatically added as Household Members but require notification to Qavvik Air within 30 days from the date of birth to continue coverage.
- Membership Fees. The current annual Membership fee of $99.00 covers the Primary Member and all Household Members.
- Insurance Coverage. I agree to notify Qavvik Air of my insurance or the insurance of any other Household Members enrolled in the Membership, and to notify Qavvik Air of any changes to our insurance. This Membership is considered an insurance contract in the State of Alaska.
- Billing. If I receive medical transportation from Qavvik Air, Qavvik Air will bill my insurance or other responsible third party payor. Membership is secondary to insurance or coverage provided by another third party payor. Qavvik Air will accept the amount paid by my insurance or third party payor for any medically necessary transport. The Membership fee constitutes prepayment for the patient responsibility amount indicated by the insurance company’s explanation of benefit for all emergent and medically necessary air medical transport services provided to the closest most appropriate facility, or payment in full of patient’s responsibility if there is no applicable insurance coverage.
- Assignment of Rights. When Qavvik Air provides medical transportation, it provides value to members and incurs cost for itself. I and my Household Members hereby assign directly to Qavvik Air all rights to insurance or other payor payments that are due in order to compensate Qavvik Air for the cost it incurs in providing medical transportation service on behalf of myself or my added Household Members. I and any added Household Members will provide Qavvik Air with all necessary insurance provider or third party payor information, as well as all other information necessary to process such claims. I and any Household Member agree to turn over any payment(s) sent by insurance or third party payor for a Qavvik Air medical transport within ten (10) days of receiving payment, or as soon as reasonably possible.
- Potential Ineligibility for Individuals Covered Under Government Payor Programs or Health Share Plans. Per government regulations, individuals covered by certain government payor programs, including but not limited to Medicare, Medicaid, Tricare, etc., may not be eligible for Membership. To the extent I have coverage under any such program/plan, I agree to contact the program/plan directly to determine if I am eligible for Qavvik Air Membership prior to applying and purchasing the Membership.
- Reservation of Rights, Term of Membership, and Free-Look Period. Qavvik Air reserves the right to decline acceptance of anyone into its Membership program if they do not meet the eligibility requirements contained in this document; however, Membership benefits will be effective immediately once Qavvik Air receives both the fully completed enrollment form and accepts payment of the Membership fee(s) (the “Effective Date”), unless Qavvik Air returns or refunds the Membership fee(s) or notifies me within 30 days that the Membership fee payment has not been accepted. Membership, including membership for Household Members, lasts for one year starting on the Effective Date of the Primary Member’s membership. I understand and acknowledge that I am entitled to a free-look period of ten (10) days following Qavvik Air’s acceptance of this Membership during which I may terminate and return the Membership by providing written notice to Qavvik Air with a full refund of any air ambulance service membership fee and the Membership will be deemed to have been void ab initio. If Membership is terminated pursuant to this free-look period, no membership benefits will apply to any transports or other services rendered by Qavvik Air to either myself or any added Household Members during the free-look period.
- Renewal and Grace Period. I understand and agree that it is my responsibility to contact Qavvik Air to renew this Membership 30 days prior to the enrollment anniversary. I further understand and agree that I am responsible for providing any changes to my account or insurance benefits, or providing confirmation that no changes have occurred. If auto-renewal is requested, the Membership will renew each year on the enrollment anniversary as long as all applicable fees are timely paid and I provide Qavvik Air with any changes to my account or provide confirmation that no changes have occurred. I understand and agree that the fees set out in this Membership are subject to increase by Qavvik Air, at Qavvik Air’s sole discretion, on each enrollment anniversary. Membership benefits for the Primary Member and all Household Members will automatically expire one year following the Effective Date. However, I shall have a grace period of thirty (30) days following termination in which to pay for a renewal membership by paying Qavvik Air’s then current rate for such membership. I understand that if I or an added Household Member is transported by Qavvik Air and my Membership has expired, the Membership benefits will not apply to the transport and I and/or my Household members will be responsible for any related costs not covered by my insurance or third-party payer.
- Requirement of Medical Necessity. Membership benefits only apply to medically necessary transports, as determined by the member’s sending medical provider. Medical transports are based on medical need, not membership status, and transport will be made to the closest, medically appropriate facility as defined by Medicare rules.
- Primary Service Area. It is Qavvik Air’s primary mission to serve the air ambulance needs of the NANA Region. For this reason, Qavvik Air’s primary service area is currently defined as service originating from within the Maniilaq service area as defined by the Maniilaq Association. Qavvik Air is not currently equipped to provide air ambulance services originating outside of that area.
- No Guarantee of Transport. I understand and agree that air medical transport service by Qavvik Air cannot be guaranteed and may not be available due to various conditions, including but not limited to, weather conditions, geographic restraints, unavailability of aircraft due to maintenance or pilot shortages, lack of resources, emergency or disaster situations, legal and regulatory restraints, commitment to another patient transport(s), or transport originating outside of Qavvik Air’s primary service area.
- Other Providers – No Benefit. I understand and agree that I and my Household Members will not receive benefits of the Membership program if transportation is made by an air ambulance company other than Qavvik Air.
- Incidental Ground Transportation. Membership benefits apply to ground ambulances operated by Qavvik Air where such ground transportation services are incidental to a covered air medical transport, such as for transportation between the aircraft and hospital, and when transfer between medical facilities is necessary. I understand and agree that ground transportation service by Qavvik Air cannot be guaranteed and may not be available at all times or in all locations.
- Non-transferability. Memberships are non-transferable. Memberships can be terminated by request of the Primary Member. Memberships are nonrefundable after expiration of the free-look period.
- Attorney Fees and Costs. In the event there is any litigation regarding this Membership and Qavvik Air is deemed the prevailing party, I agree to pay Qavvik Air’s reasonable actual costs and attorney fees.
- Applicable Law. I understand that any and all matters arising out of or relating to the Qavvik Air membership program, these terms and conditions, and/or the subject matter hereof shall be governed by, construed, and enforced in accordance with the laws of the United States of America (including without limitation, the Federal Arbitration Act) and, to the extent not preempted by Federal law, the laws of the State of Alaska, without regard to conflicts or choice of law principles, regardless of the legal theory upon which such matter is asserted.
- LIMITATION OF LIABILITY. THE LIABILITY OF QAVVIK AIR, AND THE DAMAGES AVAILABLE TO A MEMBER OR A HOUSEHOLD MEMBER, FOR BREACH OF THESE TERMS AND CONDITIONS IS LIMITED TO ACTUAL DAMAGES IN AN AMOUNT NOT TO EXCEED (A) ANY AMOUNT ACTUALLY RECEIVED BY QAVVIK AIR IN VIOLATION OF THESE TERMS AND CONDITIONS AND (B) THE MEMBERSHIP FEE PAID BY THE MEMBER FOR THE APPLICABLE MEMBERSHIP TERM. IN NO EVENT SHALL QAVVIK AIR OR ANY PROVIDER BE LIABLE TO A MEMBER OR A HOUSEHOLD MEMBER UNDER THESE TERMS AND CONDITIONS PURSUANT TO ANY CONTRACT, NEGLIGENCE, STRICT LIABILITY, TORT, OR OTHER LEGAL OR EQUITABLE THEORY FOR ANY INCIDENTAL, SPECIAL OR CONSEQUENTIAL DAMAGES OF ANY NATURE WHATSOEVER, ARISING OUT OF OR IN CONNECTION WITH THE MEMBERSHIP PROGRAM OR THESE TERMS AND CONDITIONS, EVEN IF QAVVIK AIR OR A PROVIDER HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. THE MEMBER ACKNOWLEDGES AND AGREES THAT THE LIMITATIONS OF LIABILITY SET FORTH IN THESE TERMS AND CONDITIONS REFLECT AN ALLOCATION OF RISK SET FORTH IN THESE TERMS AND CONDITIONS AND THAT, IN THE ABSENCE OF SUCH LIMITATIONS, THESE TERMS AND CONDITIONS WOULD BE SUBSTANTIALLY DIFFERENT.
